Well, it depends on if you want the entry to show for just one user, or for all users.
For just one user: Create a folder in "C:\Documents and Settings\<profile>\Start Menu\Programs\"
For all users: Create folders in "C:\Documents and Settings\All Users\Start Menu\Programs"
