I have created a custom form for use with a dot maitrix tractor fed printer.
I defined the form under "Server Properties" in the WindowsXP Printers and
Faxes dialog. The form has a length of 1.5 inches (address label). I have
set the printer defaults for the printer to the custom form I created.
When I print a test page, things seem to work fine. When I print from an
application the page length of the form I created is ignored and the paper
advances as if it were 11 inches long.

What do you think?

I assume the application gives you the ability to select the custom form you
defined and you are in fact selecting it.. Chances are the driver does not
support that size. As an experiment, you might try defining another size
like 3in or 4in and see if that works. If it does, then the driver doesn't
support the 1.5in. You might can download another driver from the
manufacturer.
