The initial problem is that the custom paper size is no
longer available as a media type in the print dialog for
my HP Deskjet 722c, an older printer. I've learned that
this feature is not part of the driver that comes with
Windows XP. I've tried creating a form in the Printers &
Faxes > File > Server Properties. My problem now is
figuring out how to make use of the new form I've created.
I tried a couple of different apps, and the paper types in
the print dialog are still the same.

Any suggestions?

It 'might' be that the form size is no longer supported with the XP driver.
Try defining a custom size that currently shows up in one of your apps (ie
define a custom size 'Test Letter 8.5x11' if 'Letter 8.5 x 11' currently
displays in your app). If this works, then the other size you are defining
is not supported by the driver. If it doesn't work, it still could be the
driver. You might try looking for the latest driver from HP. I've also
noticed that sometimes custom sizes that were defined prior to instlling the
driver do not show up. Usually deleting the form and redefining it works in
this case.
