Having to restart, shut down, log off twice

Hi all,

I am not sure what cause it. I have to restart, shut down
and log off twice in order to finish the task.

After the first click, I can hear the shutting sound, but
then, the desktop stay there and I can still open other
application.

I have checked that there is no application running.

Any ideas?
