How do I keep every program I open from being added to the
start menu?

Hi,

You don't unless the install routine for the program offers that option.
Otherwise, you can just delete the shortcut after installation.

Do you mean Start menu or Taskbar ? If latter, I don't think there is a way
as it is the primary means of access if you have more than one item
activated. If someone else does know of this possibility, please post the
answer.

Perhaps you want to remove the X from 'List my most recent documents' ?
Right click taskbar--select properties--start menu tab--customize
button--advanced tab--bottom of window.
