My scanner always comes up with color 150 dpi settings. It appears it is a
Windows XP scanner control. How do I get it to come up with B&W, 400 dpi
settings as the default?

This would be a setting in your scanner's TWAIN software - absolutely
nothing to do with Windows. Suggest you RTFM or RTFH or even VTFWS.

As with most tools included with the operating system, the functionality is
very basic. Here's the URL for an article in the MS Knowledge Base that
explains the WIA limitations:

To access the extra features your scanner offers, install the software that
came with the device. Before installing, double check that the software is
marked as suitable for use with Windows XP. You may also want to check the
hardware manufacturer's website to be sure that you have the current version
of the software.
