Have you checked your BIOS settings to see if USB and
USB/DOS (Legacy Devices) is enabled? It is possible that
you got a power spike 3 months ago on the MB and that
could have scrambled or altered those settings.

Also, have you uninstalled and reinstalled the drivers?
