Video card determines capabilities, Monitor places the limits on
which of those capabilities can be used. (Maximum Resolution @
which color depth & Refresh rate).

Its a combination of both your video card and your monitors capabilites.

First the monitor sets maximum refresh rates at any resolution -
ultimately because of the highest horizontal scan frequency it can
handle. That means that low resolutions can be used at higher vertical
(and hence over all ) rates than high resolutions.

The video card then will have hardware that has a variety of possible
modes built in. Often these will be restricted by the frequency limits
of the monitor. You can see what the card supports if you go to
Display, Settings, click advanced and on the Adapter page click 'List
all modes'. But don't try to use one outside the monitor's limits.

It is rare now for the memory on the card (or the share of main memory
given to on board video chips) to place a limit on the basic resolution.
But it may limit the number of part-screens that can be held in building
up a 3D image, which can affect performance in games.
