chkdsk runs everytime I boot up

Everytime I start my machine CHDSK runs. I let it run
everytime but same problem occurs. I have defraged all 3
drives and also scheduled a thorough CHDSK on all 3
drives, but same issue, also ran the restore to before
the problem started happening still same issue.

Any help would be appreciated.
