First I woul look at the solutions proposed there:
http://v4.windowsupdate.microsoft.com/troubleshoot/

If these do not apply, you may have the problem I had
lately and which is the following:

Reviewing the various log files created during update, I
found one named xpsp1hfm.log in the WINDOWS directory.
For each failed installed it included the lines:

d:\7dd8a7a618af5dfbeb1ab3192744\xpsp1hfm.exe version
5.3:16.6
C:\WINDOWS\system32\xpsp1hfm.exe version 5.3:16.5
CopyFile(d:\7dd8a7a618af5dfbeb1ab3192744\xpsp1hfm. exe,
C:\WINDOWS\system32\xpsp1hfm.exe) failed 5
Failed to install xpsp1hfm.exe

Which meant it was not able to install the new file
xpsp1hfm.exe on top of the old one in WINDOWS\system32\.
So I went in WINDOWS\system32\ and renamed the existing
xpsp1hfm.exe into OLDxpsp1hfm.exe.
It did the trick, now the updates install fine. I then
removed the OLDxps1hfm.exe
