Old Pedant in CodingForums.com suggested a work-around: force page breaks
rather than relying on the browser / window.print / whatever to cause them.
It worked.

Forcing page breaks also fixed...

The printer is an HP Officejet 5505. Preferences are same as for other
normal printing. Note that the header is printed just fine on the second and
later pages above the first line of the table...
