Tricky, maybe a bad internal connection. ie poor electrical contact.

Windows really really slow to start
You can boot into safe mode, then use the Boot logging option.
Once fully booted examine the boot log.

I suspect its a sata drive and as such you need to either amend your AHCI
settings in the bios

any process be accessing D

Unless your PC has a bios virus, booting from the winxp to clean install
would not be affected by any virus.
Of course you cannot format via dos, niether can you format within win, you
wont see...
