Is there any way to set a header and footer that will print on all pages sent
to a given printer?

I would like to be able to set it for the printer, so that it will be printed
on every page of everything printed from any application.

That would be a function of the printer driver and all the printer drivers
I've seen defer header and footer printing to the application generating the
printout.
