Default Scrambled Icons & Sys tray

When I am in Taskbar and Start menu properties, In Custom
Notifications, all of the icons in "past items" are wrong
(they don't match up with thier program).

Also, ones that I have set to show always in the Sys
tray, somtimes don't, (even when active).

Help is appreciated and Thanks in advance!
