entire file is re-scanned for every reference by hostname because their are
too many to fit into the local DNS cache.

with no-longer-existing entries. Example- daytodaypharmacy.com
I randomly pulled out of hpHosts, and checked it. The site hasn't
been active for almost 2 years! I researched it out of curiosity.
I then checked 5 more, and 2 were obsolete. Not good.
