How do I remove and transfer an icon from my desktop to the "Start/All
Programs" menu?

Default Removing icon from desktop

Just drag the icon to the start button and drop it there. The application
will then appear at the top of the start menu. If you want to drag it to all
programs simply click on it, hold the mouse down and drag it to wherever you
want it in the all programs menu.
