Default Printer Server Problem

When working in the "Printers and Faxes" program, I need to create a new
form definition for my dot matrix printer. I Select "Server Properties"
from the file menu and add a new form on the "Forms" tab with a 7" form
height. When I go to the "Printing Prferences" and "Advanced" button on my
dot matrix printer driver, I expect to be able to select my new form from
the "Paper Size" list under "Paper/Output". However, the selections in that
list are very limited and my new form does not appear.

Why?

Anybody?
