Default Not Optimum Mode - Optimum mode 1240 x 1040 60hz

Computer Club member's computer with Windows XP Home. Service Pack 3.

No matter what, the system boots in safe mode. Shortly a small windows opens
that reads: "Not Optimum Mode - Optimum mode 1240 x 1040 60hz" with a
question mark (?) in the middle.

Soon, the screen turns black and the small window moves around as if a
screen saver. No action will have any effect. Once, the desktop appeared
again for about 5 seconds.

I have managed to run chkdsk /f with no errors.

Malware or hardware or ?
